Bridgewell Care provides a safe and supportive environment for individuals who need continued care and stability following hospitalization, institutionalization or in need of ongoing health support. Our purpose is to help clients stabilize their health, follow their treatment plans and successfully transition back into the community.

Through recuperative care housing, targeted case management, care coordination and connections to community resources, we work with clients and their healthcare teams to address both medical and social needs that affect recovery. We support members’ recovery and coordinate access to:

• Primary and specialty medical care

• Medication management and adherence

• Behavioral health and substance use recovery services

• Transportation to healthcare appointments

• Targeted case management services

• Benefits and social services

• Housing, recovery programs and community resources

• Independent-living supports

• Timely and accurate communication with healthcare providers, health plans, and community partners

• Connection to substance use disorder treatment, including medication-assisted treatment when appropriate
